Just received myT16M in the mail this morning and I'm performing some test.
I installed the KCS 3.12 firmware and the servers webgui etc came up as expected.
Under the tab System there is this option: "Keep Output After Restart"
With this enabled I expected the outputs to com up in the last known state before restart or powertoggle, but all outputs comes up in a off state

after you enabled this option. do you have power off -> power on for testing?

Just tested and verified the problem again
Setting eg. all outputs to on, and either powercycle the unit OR performing a restart from the GUI sets all outputs to low
Did you actually test this function by yourself at all ?

After your pictures of proof, I've spend quite more time to test.
I tried reset to factory and everything else, but the missing outputs after power-toggle or restart from GUI did not solve the problem with the missing outputs after restart
Suddenly after one power-toggle the unit started to behave exactly as expected, and now I can't get it to go back in the bad state again, which of course is a very good thing
I have no idea why the problem persisted for so long time, and why the unit suddenly started to behave normal
Anyway so far, I can only give my best recommendation on this really small module, and I'll need more of them soon

Just received my 2.nd T16M, and upgraded both the new and the "Old" T16M to newest rel 3.15.0
I now see the original problem on both T16M controllers, and I'm able to reproduce the problem
Simple IFTTT settings: IN1 toggle OUT1 and IN2 toggle OUT2
If I, under monitor window, set the OUT1 & OUT2 to high, and toggle contacts physically connected to IN1 & IN2 a few times, and leaves OUT1 & OUT2 to high. Performs a reset or reboot of the controller, the high state is saved, and correctly restored after a reset on OUT1 & OUT2
If I, by toggling the input, set OUT1 & OUT2 to high, and in the GUI, under monitor window, set OUT1 & OUT2 to low. Toggle the 2 inputs, by the connected switches, to high, and then reboot or reset the controller both OUT1 & OUT2 comes up low
It looks like only the state from the monitor window is saved correctly, but not the state after toggling the outputs high if it is set low in the monitor window as written just above
I can reproduce on both controllers !!
Tried to reset, power toggle etc, but I'm able to reproduce every time I try now
